Your Child’s Health

Your child’s health is important to us. We would appreciate it if all parents and guardians would ensure that emergency contact details are up to date.

Medication
A completed Authorisation to Administer Medication Form needs to be completed and signed if your child is taking medication at school for a maximum of five days. A completed Student Medications Details Form needs to be completed and signed if your child is taking medication at school on a regular basis for longer than five days.

Medication will be administered in the Staffroom at the prescribed times. Details will be documented in the Medication Registration Folder as required by the Department of Education and Early Childhood Development.

Please ensure the following:
• Always send your child’s medication clearly labeled to your child’s classroom teacher in the original packaging with the prescribed medication details printed on the front.
• Do not allow your child to keep the medication in their school bag.
• Provide staff with sufficient medication.
• Please advise the school of any changes to medication requirements signed in writing.
